To order tile from the Spalding Foundation please do the following:
  1. Fill out the order form
  2. Print the page (File -> print)
  3. Mail the printed order form along with a check or money order payable to:
    Spalding Foundation for Injured Drivers, Inc.
    1074 Sulphur Springs Road
    Owego, NY  13827
